Attention: Over the past several months, the ACR Radiation Oncology Practice Accreditation Program (ROPA) has developed a virtual site visit process to respond to travel restrictions caused by the COVID-19 pandemic. We strongly encourage that all sites participate in a virtual site survey. If the site declines to do a virtual site survey, the ACR will provide a 3-month extension and reschedule an onsite review once travel is safe.

All sites must complete this checklist before a virtual site survey is scheduled.

We strongly encourage that required accreditation forms are sent to the fax number or email address provided during the online application process. Paper forms sent through the mail may experience a processing delay.
